Thank you for nominating for the Charity champion award. You are the nominator and the organisation you are nominating is the nominee. 

To complete the nomination form you will need details (full name, email address and phone number) of a contact person at the organisation.

You will also need to include your details (full name, email address and phone number).

Please also make sure you have told the contact person that you are nominating the organisation for this award. We will also send them an email when you finish the nomination.
